New Jersey in brief. New Jersey practices send WISeR requests to Genzeon, the state’s WISeR Participant, or to Novitas Solutions, the state’s Medicare Administrative Contractor. Decisions usually come back within 3 calendar days. Only patients with Original Medicare are affected.

Who in New Jersey is affected

Any New Jersey physician or practice that delivers one of the 13 WISeR services to a patient with Original Medicare. Medicare Advantage and Railroad Medicare patients are not affected. Requests have been accepted since January 5, 2026, for services on or after January 15, 2026, and the model runs through December 31, 2031 (Operational Guide, Quick Reference and Section 1.4).

In practice, that means pain management, spine, orthopedic, urology, ENT and sleep surgery, neurology and wound care practices. The full list of services and codes is in our WISeR guide.

Where New Jersey practices send requests

You can send a request to either organization. If you send it to Novitas, Novitas forwards it to Genzeon within about a day (Section 3).

A New Jersey request checklist

  • Confirm the patient has Original Medicare, not a Medicare Advantage plan.
  • Confirm the service and code are on the WISeR list. For 62323, only when it is used as an epidural steroid injection.
  • Copy the patient’s details exactly from the Medicare card: name, MBI, date of birth and address.
  • Include the physician’s NPI and PTAN, and the facility’s if the service is done there.
  • Attach the documentation the service requires, matched to the NCD or Novitas LCD for that service.
  • Mark it initial or resubmission, and expedited only if a delay could seriously jeopardize the patient.
  • Record the UTN when the decision arrives, and put it on the claim.

When Genzeon says no

A non-affirmation cannot be appealed, but it must list every missing or noncompliant item. Fix those items and resubmit, as many times as needed, with the patient’s details and the UTN matching the first request exactly. You can ask for a peer-to-peer review with the resubmission. Appeal rights begin only if a claim is denied (Sections 4.1.2 and 10). The full process is in our WISeR guide.

Earning an exemption in New Jersey

A New Jersey doctor with at least 10 requests in an assessment period and an approval rate at or above the minimum threshold can be exempted from WISeR review for at least a year. Exemption is earned by individual NPI. Genzeon publishes its own added criteria and runs provider education on the exemption program (Section 5).

A New Jersey team behind your requests

MedMojo™ is built in New Jersey for independent specialty practices. We check each WISeR request against what that service requires before you send it, track every decision and UTN in one place, and turn a non-affirmation into a list of what to fix. And yes, we can come to your office.

About this guide. It summarizes the public CMS WISeR Model Provider and Supplier Operational Guide, version 7.0, last updated July 24, 2026, and was checked against it on October 11, 2026. CMS updates the guide; confirm against the current version before you act. Procedure codes are listed by number only. MedMojo™ is independent practice software and is not affiliated with CMS, Medicare, or any WISeR Participant or Medicare Administrative Contractor.
